SAVANNAH, Ga. – Senior CitizensInc. (SCI), a nonprofit organization dedicated to helping people age successfully received a grant from the Jack Golden Fund of The Savannah Community Foundation to provide post-operative care for older adults that included in-home care and home delivered meals.

The purpose of the grant was to determine if immediate post-operative support would reduce rehospitalization for older adults. Care and meals were provided for two weeks by SCI at no charge to older adults with little or no family support upon leaving the hospital.

“When an older adult is discharged from the hospital, they may need assistance with their post-discharge care such as preparing meals, getting to medical appointments, or other daily living activities. These details can be vital in keeping them on the path to recovery,” said Patti Lyons, SCI president. “Fortunately, because of this grant, we were able to help older adults in Chatham County and show that this type of care directly affects successful recovery. Best of all, it gave them and their family members peace of mind.”
